Private Eye Examination Form (RX06)
A new look
The AOP Spectacle Prescription and Contact Lens Specification are now available in a new improved format. They set out all the information required for optometrists and contact lens practitioners to complete and give to the patient at the end of the eye examination under the Sight Testing (Examination and Prescription) (No. 2) Regulations 1989.
We have made the following improvements to the old form:
“The prescribing and dispensing of spectacles are very closely linked and it would be in your best interests to have your spectacles dispensed where you had your eyes examined. It is often more difficult to resolve any problems you may have with your spectacles when prescribing and supply are separated.”
We have improved the quality and presentation of the form and where previously Spectacle Prescriptions and CL Specifications were on one form, they are now separate forms.
The forms, order reference RX06 (formerly RX04) cost £70.50 incl. VAT per 1,000.
Patient Record Cards
The AOP has designed and printed a record card system to assist optometrists to keep patients' records in accordance with best practice. This consists of a personalised envelope for each patient, a dispensing record and a clinical record.
