Contract of employment and contract for services

.pdf documents on this page contain a specimen contract of employment and accompanying notes and information, and a specimen contract for servces, which may be used for self-employed locums.

The specimen contract of employment sets out terms generally included in contracts of employment. It highlights those terms which must be included in a written statement of the main terms and conditions of employment, which must be given to all employees within eight weeks of starting work. It also sets out additional terms specific to the employment of optometrists and pre-registration trainees. A specimen covering letter disciplinary and grievance policy accompany the contract.

The specimen contract for services includes terms appropriate to locum optometrists who work as self-employed optometrists or dispensing opticians, hiring their services to other.

Accompanying notes provide an overview of the two contracts and there are explanatory notes specific to each.

It should be possible for the .pdf files to be copied into Word documents, but if you are unable to do so, please contact us - e-mail stevenking@aop.org.uk - and we shall send you the documents in Word format.
