GOS Warning Issued
From August 1, GOS 1 forms will, in most cases, require two signatures. Each form will have to be signed by the optometrist who has conducted the sight test and by the GOS contractor (or authorised signatory). If the optometrist and contractor (or authorised signatory) are the same person, one signature will be enough. This requirement is part of the new regulations on ophthalmic and supplementary lists.
The GOS 1 form is being redesigned by the Department of Health but will not be ready in time, despite frequent requests by the AOP and FODO. However, the current form does not provide space for two signatures. Nevertheless, the Department will require forms to have two signatures from August 1 on the existing form as an interim measure. In agreement with the Department, the AOP is advising its members to place the contractor's signature in the space currently provided at the end of the form; and the optometrist's signature and list number in the left-hand margin opposite.
